How to update your NPI information

Only the provider, or someone they authorized, can change an NPI record. It is done in NPPES and takes a few minutes.

Changing your NPI address

  1. Sign in to NPPES with your I&A login.
  2. Open your NPI record and choose to edit it.
  3. Change the practice location or mailing address, or add another practice location.
  4. Go through to the end of the application and submit it.

Most changes show on the public registry, and on this site, within a day.

Other things you can update

  • Name, after marriage or a legal name change (you'll need to give the old name too)
  • Taxonomy codes and which one is primary
  • State license numbers
  • Phone and fax numbers
  • Other identifiers such as Medicaid IDs
  • For organizations, the authorized official

The 30-day rule

Federal rules say providers must update NPPES within 30 days of a change to their required information. Out-of-date addresses also cause problems in practice: payers and pharmacies use NPPES to match claims, and mail to an old address can lead CMS to deactivate a record.

Can't sign in?

Use the I&A account recovery on the NPPES sign-in page. If you need help, the NPI Enumerator answers at 1-800-465-3203 or customerserviceNPIenumerator@cms.hhs.gov.

Updating your NPPES record does not update Medicare enrollment. Change PECOS separately.
